A grand jury hears only the State. There is no cross-examination, no defense presentation, and no judge weighing admissibility. The standard is probable cause. Unsurprisingly, indictments are returned in the overwhelming majority of cases presented.
Why the minutes still matter
Grand jury proceedings are secret, but the court minutes recording the return are not. They identify the judge, the prosecutor, and the action taken, and sometimes carry contemporaneous notations about the state of the file.
